Chronic Disease Management Weaknesses, Possible Solutions Explained At HIMSS15

In this podcast, Lumira CEO Sumit Nagpal provides an overview of Lumira and walks through its journey from Wellogic to Alere Accountable Care Solutions and ultimately, to Lumira. Nagpal, recognized as one of Health IT Outcomes’ 2014 Health IT Change Agents largely because of his leadership in trying to improve population health and clinical outcomes, details the biggest weakness in the U.S. healthcare system today when it comes to chronic disease management and how is Lumira trying to address this challenge. Nagpal played a big part in overseeing the redirection of the healthcare system in the city of Leeds in the UK, including the implementation of several new chronic disease programs. He describes these programs and what he learned through this initiative, as well as what improvements Leeds has seen since implementing the chronic disease programs. He also explains how the U.S. healthcare system compares with those in other countries from a systemic and clinical quality standpoint and what key areas of improvement need to be focused on in the U.S. Finally, Nagpal offers his opinion as to what the main ingredients necessary to achieve successful coordinated and accountable care in the U.S. are and, if done correctly, if accountable care is the answer to many of the U.S. healthcare systems’ biggest problems.
