At this phase in EHR deployment, the organization has a fully installed, hybrid medical record; some documents originate on paper and are scanned into the EHR while most are generated electronically or automatically downloaded from other systems via an interface. A combination of technologies is used to accomplish the overall goal: ubiquitous access to patient information, regardless of location—or format.
