Infor Cloverleaf Provides Trustworthy Data Integration, System Stability
Infor, a leading provider of business application software serving more than 73,000 customers, recently announced the success that Watershed Health, a solution provider that offers patient education and engagement systems to healthcare providers to help improve the quality of care and outcomes for patients with chronic diseases, has found with the Infor Cloverleaf Integration Suite to aggregate disparate information systems and data. The Infor Cloverleaf Integration Suite is a leading clinical integration and information exchange solution that helps to create a sustainable foundation for healthcare organizations to proactively manage, coordinate and share data across the care continuum.
Watershed collects data from the healthcare provider, translates the data format from the provider's electronic medical records (EMR) to a useable format, and routes that data to the appropriate Watershed system for risk stratification and analysis on the patient's condition, and to develop a plan to improve outcomes. Before Watershed could begin to work with providers, it needed a partner to seamlessly integrate data from the provider's systems to theirs - and Infor Cloverleaf was recommended to them by multiple hospitals as a trustworthy data integration engine.

Specifically, Watershed decided to move forward with Infor because of the application's ability to limit risk, allowing the organization to react to different client needs and requirements, and do so with a standardized approach that reduces overall implementation time. In addition, the solution reduces hospital involvement, and Watershed can reach out over the internet and gather data securely-with little assistance from the hospital-to bring data back into the Watershed network for analysis - streamlining the process, and saving time and administrative work.

So How Does it Work?
Watershed began working with Springhill Hospital, Mobile, Ala., as a client and the two organizations initially focused on heart failure patients. Once a patient admits to Springhill, patient data migrates into the Watershed system via Infor Cloverleaf. Patients answer questions to identify risk factors on iPads® before they leave the hospital. After the patient is discharged, Watershed continues to follow the patient's progress for the next 30 days.
Since the care transition from hospital to home may cause problems with medication adherence, missed follow-up appointments, and forgotten protocol recommendations, that 30-day post-discharge period plays a pivotal role in the patient's recovery and ability to administer self-care.
During the 30-day post-discharge period, patients who require weight monitoring weigh themselves with wireless scales, and the scales generate weight reading biometrics that go directly to Watershed. If weight gain occurs, the Watershed system alerts users. Heart patients who gain weight may develop breathing difficulties, which may prompt hospital readmissions. Catching that weight gain and managing it early can help reduce hospital readmissions.
According to Watershed, since starting the program Springhill has reduced hospital readmissions for heart failure patients by more than 50 percent.
