Understanding And Implementing Population Health Strategies
As the national health system shifts to “value-based care,” it is imperative that all roles within the system understand population health management and various strategies to assure that clinical and fiscal outcome goals are met. A review of the drivers of healthcare reform will enlighten the need for healthcare organizations to collaborate and innovate in new ways to achieve the Triple Aim of lower cost, better quality, and an improved patient care experience.
While the role of health information technology (HIT) and clinical analytics are key in managing population health, other critical factors include patient and provider engagement. This requires new skills and methodologies to engage the entire healthcare ecosystem in providing care across the continuum. Examples of national and regional efforts to standardize and analyze care to improve population health management will be shared.
