HealthCare Partners looks to ensure quality, access, and affordable care for all with implementation of EHR, patient portal, and population health management solution

HealthCare Partners, LLC, based in Torrance, CA, is deploying Allscripts Enterprise EHR across all 65 medical offices within its California HealthCare Partners Medical Group. In addition, the management services organization has implemented Allscripts solutions, FollowMyHealth patient portal and dbMotion population health management. The solutions expand a nationwide partnership for enterprise-wide integrated healthcare delivery.

Specifically for large, multi-specialty practices, Allscripts Enterprise is an ambulatory EHR that will create continuity of care across multiple settings, leading to better patient care and outcomes. Complementing the easily accessible patient records is HealthCare Partners’ adoption of the FollowMyHealth patient engagement system. The online patient portal will enable patients to view their personal health record and become more active in managing their own care and taking preventative measures.

“Our vision is to be the role model for integrated and coordinated care, leading the transformation of the national healthcare delivery system to ensure quality, access, and affordable care for all,” said Zan Calhoun, CIO of HealthCare Partners, LLC. “Our decision to use the Allscripts Electronic Health Record enterprise-wide in addition to our selection of FollowMyHealth for patient engagement and dbMotion for connectivity, will further enable us to do just that.”

The Allscripts dbMotion platform will deliver interoperability for the organization. Utilizing the solution will enable HealthCare Partners to manage population health enterprise-wide. “We are pleased that HealthCare Partners has decided to expand its commitment to Allscripts as part of its unwavering focus on patient-centered care,” said Paul M. Black, president and CEO of Allscripts. “The combination of our Open Electronic Health Record, patient engagement portal, and community interoperability solutions will empower HealthCare Partners to continue as a leader in this evolving healthcare environment and better manage population health for the communities it serves.”

A subsidiary of DaVita HealthCare Partners, HealthCare Partners, LLC, manages the administrative systems of its staff model, HealthCare Partners Medical Group. Employing more than 600 primary care physicians and specialists, the medical group has been recognized for the past ten years by the Integrated HealthCare Association (IHA) for clinical quality, patient experience, use of information technology-enabled systems, and coordinated diabetes care. The medical group is one of four organizations to participate in the Brookings-Dartmouth Accountable Care Organization pilot and serves residents of Los Angeles, Pasadena/San Gabriel Valley, South Bay, Long Beach, San Fernando/Santa Clarita Valleys, and Orange County.

Additionally, HealthCare Partners, LLC, provides support services to its Independent Physician Association (IPA) model with more than 1,400 contracted primary care physicians and 3,000 contracted specialists in Arizona, California, Florida, Nevada, and New Mexico.
