The Polyclinic Selects Phytel And Verisk Health For Population Health Management

Multi-specialty clinic will adopt combined Phytel and Verisk Health technologies to drive successful participation in the Medicare Shared Savings Program and commercial risk-based contracts

The Polyclinic, one of the largest multi-specialty clinics in the Puget Sound region, has selected Phytel's Population Health Management solution and Verisk Health's Provider Intelligence solution to deliver a population health management platform that enables its more than 200 physicians to deliver higher-quality care at a lower cost.

The Polyclinic is a pioneer in the healthcare industry's transition from a fee-for-service to a value-based payment model in which providers are paid according to the quality of care they deliver. The clinic has partnered both with commercial payers on such contracts and with the Centers for Medicare and Medicaid Services (CMS) as an early participant in the Medicare Shared Savings Program. The solutions from Phytel and Verisk Health will deliver a key platform that links financial and performance analytics with case management and population health tools within existing care delivery models.

"Fundamental to becoming a value-based provider of healthcare services is the ability to collect, organize, and mine clinical data in a way that reliably allows evidenced-based care to be delivered in a patient-centered, timely, efficient, and cost-effective manner across a heterogeneous patient population," said Dr. Michael Tronolone, chief medical officer of The Polyclinic."We see our implementation of the Phytel-Verisk platform as a big step toward achieving this capability."

Phytel CEO Steve Schelhammer said, "The Polyclinic has committed to making value-based care a reality. We are confident our population health management tools, in combination with Verisk Health's solutions, will give The Polyclinic the range of insight it needs to improve system performance and quality of care while reducing costs across its entire patient population."

Phytel's Population Health Management solution aggregates data from electronic health records and practice management systems in hundreds of physician groups nationwide and offers a suite of tools for automating population health management, including modalities for patient outreach, care gap identification, risk stratification, care management, patient engagement, transitions of care, and performance evaluation. Verisk Health's Provider Intelligence helps identify and target inappropriate use of medical resources, manage in-network and out-of-network costs and utilization, and drive physician and practice-level strategies focused on improving the patient experience and maintaining business stability and profitability.

Combined, these services will deliver a key platform that links financial and performance analytics with case management and population health tools within existing care delivery models. The resulting solution will provide healthcare organizations the ability to identify and address high-cost or low-performing areas within their organization while engaging patients to improve the health of their entire population.

"To succeed in the Shared Savings Program, we need fully developed and automated population health management capabilities that combine financial analytics, risk assessment, and quality improvement with care management tools," commented Denis McDonald, senior vice president of Business Services for The Polyclinic. "In Phytel and Verisk Health, we have found a solution that immediately furthers our strategy for meeting CMS requirements and realizing shared savings."

Jim Bogdan, senior vice president of Provider Solutions at Verisk Health, said, "By combining the significant capabilities of these two companies, providers can implement quality-based initiatives that drive cost-effective care delivery and improve system performance. These data-driven initiatives are a critical step toward improving a population's well-being and reducing costs associated with gaps in care, uncoordinated service delivery, and unnecessary utilization."
