Phytel Invited To Showcase Its Population Health Solutions At Health 2.0

Will Demonstrate How Consumer E-Health Devices and Applications Can Be Integrated With Phytel's Population Health Management Suite

Phytel, the leader in automated, provider-led population health improvement, has been asked to present at the upcoming Health 2.0 conference, which will take place October 7-10 at the Hilton Union Square in San Francisco. Phytel's executives will showcase the company's cutting-edge population health management solutions, which are being expanded to take advantage of the latest advances in consumer e-health capabilities.

Phytel CEO Steve Schelhammer has been selected for the October 9 forum on key issues in health IT as part of the keynote presentation. Of the hundreds of health IT executives who will attend Health 2.0, Schelhammer is one of just three CEOs who have been chosen to take part in this important event. Journalists will interview three industry leaders in 15-minute segments on how their companies are transforming, and being transformed by, Health 2.0.

Immediately following that session, Phytel will demonstrate its solution within the "Care Delivery Transformed by Technology" seminar. The interactive panel discussion will explore how Phytel's latest technologies are empowering the new care delivery models, including accountable care organizations and patient-centered medical homes.

Phytel will demonstrate how its Atmosphere enterprise care management platform can be integrated with consumer e-health devices and applications, using an open API. As the company continues to integrate patient engagement applications into its Phytel Engage services, the platform's automated architecture will use data from devices and smart phones to help identify patients with a propensity for health problems such as obesity and high blood pressure. Phytel will be able to analyze these metrics and directly communicate with a consumer's caregivers. The flexibility of the Phytel platform will allow care managers to focus on data for specific cohorts and tailor their interventions for individual patients.
