The i2i Tracks Population Health Management Solution is being utilized in all of CareSouth Carolina’s facilities.

The i2i Tracks Population Health Management Solutions program from i2i Systems has been selected by CareSouth Carolina for use in all the organization’s facilities.

The i2i Tracks solution helps standardize data from disparate electronic health records, practice management, and other systems which simplifies “the path to actionable information for an entire patient population.” As a result, clinicians and their teams are able to engage in proactive, rather than reactive, patient management, the news release continues, which increases staff efficiency and enhances clinical outcomes.

As part of the i2i Tracks implementation, CareSouth Carolina redesigned their care teams, adding community health workers and certified medical assistants to the staff. All care providers are trained on the i2i Tracks system.

“i2iTracks is definitely transforming our ability to provide quality care for our patients,” said Randy Carlyle, Director of Health Services at CareSouth Carolina. “With i2iTracks we can build a custom report in just a few minutes, and I can easily show anyone on our staff how to run a report. The addition of i2i has transformed the integration of support services and improvements in process and outcome measures for our patients. Everything from tracking and follow-up for well child visits and diabetic and hypertension wellness services to prenatal care, ER utilization, follow-ups and tons of others have all been dramatically accelerated. It has been tremendous.”

The i2i Tracks Population Health Management Solutions program is used in more than 1,000 healthcare facilities in 35 states. As a company, i2i Systems focuses on population health management, offering solutions to health center-controlled networks, primary care associations, physician group practices, hospitals, medical colleges, large employers, community health centers, public health departments, and health plans.

CareSouth Carolina offers primary care in a number of offices across South Carolina. The organization also provides specialty care in a number of areas, including dentistry, HIV/AIDS, and pediatrics.
