New York DSRIP Selects PHM Solution

The two technologies from eClinicalWorks will provide analytics and help to create a community health record.

Advocate Community Providers (ACP), the largest Delivery System Reform Incentive Payment (DSRIP) program in the state of New York, has selected eClinicalWorks Care Coordination Medical Record (CCMR) for population health management and the eClinicalWorks Electronic Health eXchange (eEHX) interoperability platform for medical records.

The first of the two technologies, CCMR, provides analytics on cost utilization, clinical quality measures, and population health. The eEHX, the second eClinicalWorks technology selected by ACP, is a health information exchange that will connect a number of different systems, leading to the creation of a community health record that will provide access to patient data and allow for a more effective exchange of data between physicians.

“DSRIP is an innovative program for reducing unnecessary hospital visits along with improving patient outcomes,” said Girish Navani, CEO and co-founder of eClinicalWorks. “Our CCMR population health platform and health information exchange will deliver a longitudinal view of ACP patients with … analytics that are actionable at the point of care. These … will benefit organizations in pursuing the DSRIP program along with several other quality initiatives.”

The eEHX is a vendor-neutral open network that connects ambulatory EHRs with in-patient hospital systems and payer claim feeds, including Medicaid, to provide cost and utilization for patient services. As a company, eClinicalWorks aims to extend EHRs beyond physician offices and into the community.

The technologies available to ACP through eClinicalWorks will help address the objectives of the DSRIP program for the organization’s 437,000 patients. The DSRIP program is a re-investment in the state’s Medicaid program that has a primary goal of reducing avoidable hospital use by 25 percent during the next five years. Nearly $6.5 billion has been allocated to the program; payouts are based on achieving pre-defined results in system transformation, clinical management, and population health.

One requirement of the DSRIP program in the state of New York is that organizations are recognized as a Level 3 Patient-Centered Medical Home (PCMH). The solutions from eClinicalWorks are pre-validated by the National Committee for Quality Assurance and provide organizations with 16.25 auto-credit points toward PCMH recognition. Nearly three-quarters of Medical Homes using eClinicalWorks (70 percent) have achieved Level 3 recognition, the highest level possible.
