Lahey Health Selects Phytel For Population Health Management To Help Drive Patient-Physician Partnerships And Accountable Care

Technology will support Lahey Accountable Care Organization (ACO) through patient engagement, quality improvement, data integration

Phytel recently announced that Lahey Health has selected the company's comprehensive, Web-based population health and population engagement platform to help keep patients healthy, minimize expense and drive better outcomes.

Phytel's technology is used by hundreds of large physician groups and Accountable Care Organizations (ACOs) across the U.S. to enable timely provider-led interventions that ensure patients get the care they need when they need it. Lahey Health launched the Lahey Clinical Performance ACO in 2013 to deliver seamless, high-quality care for Medicare beneficiaries.
