Hillcrest HealthCare System Selects Phytel For Population Health Management
Phytel, the leader in automated, provider-led population health improvement, announced recently that Hillcrest HealthCare System (HHS), a network of six hospitals and 49 clinics in northeast Oklahoma, has selected Phytel to help HHS' physician group execute its population health management strategy.
HHS will deploy Phytel Outreach to 100 primary care physicians--nearly half of the 195 providers at Utica Park Clinic--to ensure that their patients receive recommended preventive and chronic care. The health system then plans to implement Phytel Insight, which uses a web-based registry and advanced analytics to measure the effectiveness of quality initiatives and generate actionable data to improve health outcomes across a population.
Hillcrest is involved in multiple quality improvement projects with both commercial health plans and Medicare. Sixty Utica Park Clinic physicians were recently accepted into Medicare's Comprehensive Primary Care Initiative (CPCI), which gives primary care physicians the resources they need to better coordinate care for Medicare patients.
