Ensocare And University Of Iowa Hospital And Clinics To Present Best Practices For Post-Acute Care Coordination At HFMA ANI 2014

Wayne Sensor, CEO of Ensocare, and Josh Brewster, director of social services at University of Iowa Hospitals & Clinics, will lead a presentation titled, “Using Your Post-Acute Network to Reduce Readmission Penalties” on Monday, June 23rd from 2:15-3:30 pm PST. During the session, Sensor and Brewster will detail best practices for optimizing post-acute networks in order to reduce readmissions, improve patient and family engagement, and enhance care team coordination post-discharge.
