Cloud Based Population Health Management

Single EHR platform selected by New Jersey health network to unify its 45 medical practices.

CarePoint Health, of New Jersey, has selected eClinicalWorks electronic health record (EHR) and Care Coordination Medical Records (CCMR) for its 45 medical practices. The implementation is expected to streamline processes, improve continuity of care, reduce costs, and improve outcomes.

With Meditech inpatient systems in its three hospitals, CarePoint sought to improve care coordination with a single EHR platform. “Focusing on preventive medicine, healthcare education, and disease management, CarePoint Health System excels in providing patients with 360-Degree Coordinated Care,” said Joel Taylor, corporate CIO of CarePoint Health System. “We searched for a solution that could help raise consistency across the continuum, increase revenue capture, and, most importantly, improve quality of care and patient safety. In addition to being able to aid us in meeting these goals, our physicians found eClinicalWorks’ interface the most user-friendly and the continuity of care was the best they had seen.”

A leader in ambulatory healthcare IT solutions, eClinicalWorks’ cloud-based platform connects disparate systems, ensuring that critical patient information is available at the point of care across multiple settings. With a focus on preventative care, CarePoint expects to contain costs and improve the health of its population. “CarePoint Health System is a leading organization in patient care and continues improving its system,” said Girish Navani, CEO and co-founder of eClinicalWorks. “We recognize that electronic health records systems need population health functionality in order to adequately meet healthcare’s evolving requirements, and have built capabilities that will propel organizations, including CarePoint, ahead.”

CarePoint Health is an integrated health network of three hospitals and 45 medical practices serving residents of Hudson County, NJ. Patient-focused 360 Degree Coordinated Care delivered at CarePoint facilities has resulted in recognition that includes the Joint Commission’s Gold Seal of Approval, “A” grades from The Leapfrog Group, and inclusion in U.S. News and World Report’s Best Hospitals.
