Wellcentive Introduces Wellcentive Advance™ Risk Manager

Wellcentive proudly announces the introduction of Risk Manager, a predictive risk modeling system within its Advance™ solution. Risk Manager delivers a key additional element of a responsible, end-to-end population health management solution by providing a quantitative picture of potential health risks and cost drivers within a selected population of patients. Users can identify and stratify patients within selected risk profiles, enabling providers to focus their care management resources on the highest risk patients. This results in a greater ability to effect change in both the quality of care and the costs associated with that care.

With Risk Manager, providers and their organization can now look forward – as well as retrospectively – and employ predictive risk modeling that analyzes patient data to project clinical and financial outcomes. With Wellcentive Advance Risk Manager, providers can use this information to take proactive action to address issues associated with high-risk patients to better manage their care.

To deliver these capabilities, Wellcentive has seamlessly incorporated The Johns Hopkins ACG® System into the Wellcentive Advance platform. The ACG System measures the morbidity burden of patient populations based on disease patterns, age and gender, taking a “person-focused” approach that allows it to capture the multidimensional nature of an individual’s health over time.
