Fort Drum Regional Health Planning Organization Selects Wellcentive To Provide Regional Population Health Management Solution

Wellcentive Advance™ to Provide Disease Registry, Case Management and Predictive Analytics for North New York County Region

Wellcentive is proud to announce an agreement with Fort Drum Regional Health Planning Organization (FDRHPO) to develop a disease registry to track and monitor COPD in their region by the end of 2012.

Serving the military and civilian populations of Jefferson, Lewis and southern St. Lawrence counties, FDRHPO provides a platform that analyzes existing healthcare delivery options and seeks new opportunities to leverage healthcare resources. This approach is expected to significantly strengthen the healthcare system, which serves more than 44,000 DoD beneficiaries and 120,000 civilian residents in the Fort Drum health service area.

Wellcentive Advance will enable FDRHPO to access regional and community level data via reporting and care alerts. This access and visibility will provide greater ability to monitor patient care needs and to proactively target providers and patients – all with a single goal of improving the value and efficiency of care and medical outcomes. Once the Wellcentive solution is in place to help collect, report and make informed population health decisions regarding COPD, FDRHPO plans to expand the program to other chronic diseases in 2013 and beyond.
