Actionable Care Coordination Impacts Chronic Kidney Disease Communities

Chronic Kidney Disease is a major problem. 31,661,648 people in the US currently suffer from CKD or End Stage Renal Disease. For these people, a major problem both, for them and our health care system, is re-hospitalization.

A person can miss a dialysis appointment and end up in the ER. In many cases, the patient's care team is not notified of that trip. If they were, the care team could simply go to the ER to administer a dialysis appointment right there. Unfortunately, this does not always happen and can lead to huge medical bills. The average re-hospitalization costs $20,794.

And, 1 in 4 people suffering from CKD have an unplanned re-admission to the hospital within 30 days.
