Wellcentive Chosen By The Physician Alliance To Help Physicians Improve Quality Of Care

Wellcentive, an Atlanta-based provider of innovative Population Health Management and Data Analytics solutions with a national footprint, announced the expanded use of Wellcentive by The Physician Alliance in St. Clair Shores, Mich. to manage a wide range of quality improvement and data analytics activities.

The Physician Alliance (TPA), one of Michigan’s largest physician organizations with more than 2,200 physicians, will deploy Wellcentive Advance to create a comprehensive quality improvement registry to help streamline the ability to capture, aggregate, manage and report critical patient and physician data. Wellcentive’s robust data aggregation platform allows TPA to integrate patient data from multiple electronic medical records and other HIT systems into a central repository.

The registry can flag patients who meet certain criteria, search for specific diagnoses, report specific measures in chronic diseases and more. The capabilities delivered by Wellcentive will allow TPA to proactively monitor clinical goals for its entire patient population for chronic disease management and wellness, and participate in various pay-for-performance programs available through Michigan payers. TPA will also exclusively employ Wellcentive’s Outcomes Manager to aggregate, analyze and activate information from complex and disparate programs, providers, practices, hospitals and EMRs for its numerous quality programs.
