Patient Roadmap For ACOs

Source: Greenway Health

Patients and the quality of care they reserve lie at the heart of the mission of accountable care and ACOs. As an active participant within the ACO structure and with their own health, they must be educated to maximize the benefit offered to them.

  • Fee-‐for-‐service Medicare patients who see providers that are participating in a Medicare ACO maintain all of their Medicare rights
    • Includes right to see any provider that accepts Medicare, regardless of whether or not they participate in an ACO
    • Patients will not enroll in an ACO and will not be locked into a single provider
    • Patient assignment in an ACO will not affect the way they seek care or benefit design
  • Medicare beneficiaries without a PCP, however, might be assigned to an ACO on the basis of primary care services provided by other physicians
  • Focus will increasingly be on patients as incentives align with quality outcomes
