Configured and launched in less than one month, CERECONS' ACO Startup Solution is a platform that will empower AppleCare Medical ACO to proactively manage patient care and cost for nearly 8,000 Medicare beneficiaries. In addition, the platform provides the care coordination functionality and features required by CMS to qualify for payment under the Medicare Shared Savings Program.
