AFrame Digital Selected As Preferred Patient Telemonitoring Solution For Virginia Hospitals

The Virginia Hospital & Healthcare Association (VHHA), through its shared services affiliate, Virginia Hospital Shared Services Corporation, (“VHHA Services”), announced its selection of AFrame Digital, Inc. as its preferred patient telemonitoring solution for Virginia’s hospitals and health systems.

“AFrame Digital’s ease-of-installation and use in both health care facilities and home-based settings stood out in our review of patient telemonitoring solutions. The technology’s broad range of capabilities will help health care providers engage high-acuity/high-risk patients and their families, improve care coordination, and optimize the allocation of our members’ clinical resources,” stated VHHA Services’ Vice President David Jenkins.

MobileCare Monitor currently integrates telehealth, activity and patient-reported survey and medication compliance data on a single FDA-cleared platform to help care teams quickly assess each patient’s health and well-being.  Beneath an intuitive web interface lies a data analytic engine that continuously and non-intrusively evaluates the wellness of monitored individuals in real time, based on selected parameters.  MobileCare Monitor also issues mobile alerts when it detects data that deviate from a patient’s stated targets, personal baselines or goals.  A wristwatch-based monitor establishes a 24/7 electronic safety net, generating alerts for fall-related impacts or help requests.  “Because the watch form factor is less stigmatizing, frail patients are more likely to wear the monitor when they need help,” explains Jill DeGraff Thorpe, AFrame Digital’s Vice President for Strategic Initiatives, adding “It also means that the monitor is able to gather activity data, which help care teams evaluate a person’s daytime and nighttime activity patterns.” MobileCare Monitor’s platform is SaaS-based, standards-based, device agnostic and interoperable with other applications.

“We selected AFrame Digital to leverage the power of real-time data analytics from home-based monitoring systems, in much the way we are doing with clinical data,” says David Adams, Chief Strategy Officer, Centra Health, which is using MobileCare Monitor in its PACE and home health programs.  “We believe AFrame Digital has developed a platform that addresses our goal of building scalable care management practices and helps hospitals transition to population health models of care.”
