WPH will demonstrate how they leveraged a rapid implementation process to efficiently deploy patient-centered care plans within their EHR. This process included forming an interdisciplinary care planning council consisting of respiratory, nursing, informatics, nutrition, physical therapy, care management, and others to customize an astounding 55 care plans in just 3 days based on the current and most relevant evidence.
As a result of their efforts, care plan utilization increased from just 58% to 95%. User satisfaction increased as well from both novice and veteran clinicians.
