Transforming Healthcare Delivery System With Deployment Of EHR, Patient Portal

HealthCare Partners looks to ensure quality, access, and affordable care for all with implementation of EHR, patient portal, and population health management solution

HealthCare Partners, LLC, based in Torrance, CA, is deploying Allscripts Enterprise EHR across all 65 medical offices within its California HealthCare Partners Medical Group. In addition, the management services organization has implemented Allscripts solutions, FollowMyHealth patient portal and dbMotion population health management. The solutions expand a nationwide partnership for enterprise-wide integrated healthcare delivery.

Specifically for large, multi-specialty practices, Allscripts Enterprise is an ambulatory EHR that will create continuity of care across multiple settings, leading to better patient care and outcomes. Complementing the easily accessible patient records is HealthCare Partners’ adoption of the FollowMyHealth patient engagement system. The online patient portal will enable patients to view their personal health record and become more active in managing their own care and taking preventative measures.

The Allscripts dbMotion platform will deliver interoperability for the organization.
