Telehealth Solution Remotely Monitors Patients

Florida Hospital Memorial Medical Center implements telehealth solution to provide clinical services to patients remotely

Florida Hospital Memorial Medical Center, of nonprofit healthcare leader Florida Hospital, has contracted with Authentidate to employ InscrybeMD telehealth solution to remotely monitor patients. The implementation is expected to improve patient status, reduce ER visits and readmissions, and increase patient satisfaction.

Florida Hospital sought a method to provide follow-up care for patients who are managing chronic diseases and conditions such as chronic heart failure (CHF), chronic obstructive pulmonary disease (COPD), diabetes, and hypertension. Florida Hospital Community Care, a partnership with Bethune-Cookman University, was piloted in which clinicians, with support from student health coaches, coordinated care for the patient while at home.

With the secure InscrybeMD web application, providers can view a patient’s vitals and make adjustments to care plans. These are communicated to the patient via the Electronic House Call device (EHC) with touch screen during scheduled sessions. In addition, disease management education and automated reminders for diet, medication, and follow-up physician visits all increase patient compliance and encourage healthier habits, leading to overall improved health.
