PHM Improves Quality Of Care In South Carolina

The i2i Tracks Population Health Management Solution is being utilized in all of CareSouth Carolina’s facilities.

The i2i Tracks Population Health Management Solutions program from i2i Systems has been selected by CareSouth Carolina for use in all the organization’s facilities.

The i2i Tracks solution helps standardize data from disparate electronic health records, practice management, and other systems which simplifies “the path to actionable information for an entire patient population.” As a result, clinicians and their teams are able to engage in proactive, rather than reactive, patient management, the news release continues, which increases staff efficiency and enhances clinical outcomes.

As part of the i2i Tracks implementation, CareSouth Carolina redesigned their care teams, adding community health workers and certified medical assistants to the staff. All care providers are trained on the i2i Tracks system.
