State-wide Arkansas non-profit health centers use EHR-agnostic, population health management solution for improved coordinated care.

Community Health Centers of Arkansas, Inc. (CHCA) a state-wide network of community health centers, has selected the eClinicalWorks Care Coordination Medical Record (CCMR) solution for its 122 member providers across 10 practices and 58 locations. CHCA intends to use CCMR for coordinating care across the continuum including several electronic health records (EHR) systems evaluating population health and quality improvement activities.

CHCA is a non-profit organization that focuses on fostering access to comprehensive, affordable, accessible, and quality primary and preventive health care services for underserved Arkansans. CHCA facilitates shared resources, collaborative partnerships, technical assistance, and training focused on helping Community Health Center members strengthen and expand its services.

In a release from eClinicalWorks about the partnership, Mary Leath, CEO of CHCA notes, “The organization was searching for a population health management solution that is EHR agnostic while giving us and our providers a complete view into our patient populations. eClinicalWorks CCMR will give us that clarity and presents our community health centers with actionable information as needed for a wide variety of activities.”

As a vendor to more than 450 Federally Qualified Health Centers/Community Health Centers (FQHC/CHCs) throughout the country, eClinicalWorks delivers quality healthcare to medically underserved and low income populations.

eClinicalWorks CCMR provides visibility into patient care across all settings, allowing for smooth transitions of care while improving population outcomes and cost reductions. Cost utilization dashboards allow organizations to gain insight into the total cost spent on inpatient/outpatient, pharmacy, and DME services. CCMR helps firms identify high-cost/high-risk beneficiaries and help case managers and providers create better care plans using closed-loop analytics.

It’s expected that CHCA will use CCMR to further engage patients through patient apps on their smartphones, communicating with patients through voice, text, or app notifications and receive patient responses at the point of care.

CCMR’s community integration platform manages the secure exchange of data between ancillary service providers (labs, radiology, and hospital information systems), local, state, and regional health information exchanges (HIEs), immunization registries, and ambulatory EMR systems.
