New York State Department Of Health Announces THINC As A Contractor For Population Health Improvement Program (PHIP)

The New York State Department of Health (NYS DOH) announced Friday, December 5th the selected regional contractors for the Population Health Improvement Program (PHIP), a state program designed to promote the Institute for Healthcare Improvement’s “Triple Aim” of improving patient care, achieving better population health, and lowering health care costs. The NYS DOH has divided the PHIP work into eleven regions where PHIP awardees will work alongside community stakeholders in developing innovative health promotion strategies.

It was announced on December 5th that the recommended $2.4 million PHIP award for the Mid-Hudson Region was granted to Taconic Health Information Network and Community (THINC).

The PHIP is a data-driven effort that will bring together a variety of stakeholders that impact, or are impacted by, healthcare issues for the purpose of addressing population health and disparities. As a PHIP contractor, THINC will be responsible for organizing meetings of local stakeholders and providing a neutral forum to help identify, share, disseminate, and implement best practices and strategies that will promote better health outcomes and reduce disparities across the region.

“THINC has a history of supporting innovative healthcare initiatives in the region and managing large scale projects. We look forward to working with Hudson Valley stakeholders on the PHIP aims, including analyzing data and identifying regional opportunities to address population health and health disparities. We will be encouraging all interested stakeholders to participate,” said Susan Stuard, THINC’s Executive Director.

The recommended PHIP contractor awards are to be paid out over the course of the next two years, contingent upon availability of funding and approval by the New York State Attorney General and Comptroller. Read more here(http://www.health.ny.gov/community/programs/population_health_improvement/).
