HIT Partnership Yields Comprehensive Patient Engagement Tool

Predictive analytics business adds health literacy tool for newly discharged patients.

Health IT company RightCare, which developed a way to identify and track patients at risk for readmission, will collaborate with Emmi Solutions  to offer a content tool for patients according to a company statement. The deal is designed to help patients better understand their care plan and condition with the goal of improving interactions between patients and providers to cut readmission risk.

The new product will be labeled RightCall and it is anticipated the added communication will enable providers and case managers the ability to anticipate potential readmissions, providing the necessary time to advise the patient and offer a solution – ultimately preventing a readmission.

Houston Methodist Hospital is already using both Emmi Solutions and RightCare products together. “We have been working individually with both companies for distinct purposes, so it’s exciting to watch this partnership form. We have been using EmmiTransition in our psychiatric department to help guide patients through the care transitions process, and we recently implemented RightCare’s full suite of software to reduce readmissions among our mental health population,” said Heather Chung, Director of Behavioral Health Transition of Care Program from Houston Methodist Hospital.

One objective of the software is to ensure patients get follow-up care and steers them to schedule follow-on appointments. “RightCare’s mission to reduce readmissions and improve outcomes aligns with our own and we welcome the opportunity to collaborate,” said Devin Gross, CEO of Emmi Solutions. “Beyond delivering recovery instructions, our call campaigns track patient feedback and facilitate targeted outreach, enabling RightCare to optimize their solution, and offer case managers the ability to reach out to patients at risk for readmission who require intervention the most.”
