Health System Selects PHM Solution

The system will provide Southeastern Health with a tool for population health management.

Southeastern Health, a non-profit healthcare system in North Carolina, has implemented the Care Coordination Medical Record (CCMR) from eClinicalWorks for population health management. The CCMR from eClinicalWorks is a digital platform that provides analytics for a number of facets of care, including cost utilization and measures of clinical quality.

The CCMR allows organizations to examine population health via zip code and region and can facilitate examination of local results compared to national trends. In addition, the program provides population health alerts that can be configured by the community. The CCMR operates on a vendor-neutral open network that securely connects ambulatory EHRs with hospital in-patient systems and payer claim feeds, including the Centers for Medicare & Medicaid Services, according to a news release.

According to the news release, the CCMR allows Southeastern Health “to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, and engage patients and providers in preventative care, improving outcomes among populations and reduce costs.”

In addition to the CCMR, eClinicalWorks provides EHR solutions, tools for revenue cycle management, and patient engagement. The privately held company has an established customer base of more than 100,000 physicians and 600,000 users throughout the United States.

Southeastern Health is a non-profit health organization that offers a combination of acute, intensive, and psychiatric services to more than 16,000 inpatients and 68,000 emergency patients annually.
