ACO Implements PHM Solution

Emerald Physicians ACO of Hyannis, MA, has chosen eClinicalWorks Care Coordination Medical Records (CCMR) to advance its physician-led ACO objectives. The implementation is expected to aid in care coordination, population health management, and quality improvement.

Following the ACO delivery model and a participant in the Medicare Shared Services Program of the Centers for Medicare & Medicaid Services (CMS), Emerald Physicians ACO sought to strengthen the relationship between its patients and healthcare team. The eClinicalWorks solution with its open network enhances coordination between providers, improving the quality of care for Medicare Fee-For-Service (FFS) beneficiaries while increasing satisfaction and lowering costs.

 Emerald Physicians ACO recently became a member of Cape Cod Healthcare, with two acute care hospitals – 259-bed Cape Cod Hospital in Hyannis and 95-bed Falmouth Hospital – and two-time winner of Truven Analytics’ Top 15 Health Systems study. The internal medicine and specialty care practice operates nine locations across Cape Cod with over 150 employees including physicians, physician assistants, nurse practitioners, and support staff. Serving over 38,000 patients since 1997, the practice offers services in neurology, women’s health, and dermatology, in addition to in-house lab, bone density radiology, pulmonary function test, EKG, joint injection, and home sleep study. A newer service of the progressive medical practice is home visits for patients who are homebound.
